If accepted, you will be asked for a $300 deposit to secure your spot on a waitlist - You will be guaranteed an appointment, just not given a date yet. The expected wait time if accepted is roughly 1–3 months. If you are not comfortable leaving a deposit without a set date immediately, please do not proceed with the form.
